So this is an interesting problem, at least for me, and I hope that someone has a solution.
My wife tells me that something looks different on her iPhone 4 screen. I take a look and I see that the Nav Menu that usually has the Phone, Mail, Safari and Music apps by default is missing the the Phone app. I looked at other screens and it wasn't moved. I think it was accidenyly deleted. The question is, how do I get it back?

You can't delete it. Look on all your screens and inside all your folders. If you still can't find it go to Settings>General>Reset>Reset Home Screen Layout. This will restore the home screen to its original configuration but may move other apps around to do so. You may want to take screenshots of each of you screens before doing this so you can use them to re-create your app arrangement.
