My iPhone 4s randomly exits out of apps or texts back to homescreen what can be done to fix this

My phone while texting or in an app will randomly exit back to the home screen. This gets very annoying. Has anyone had this same problem and if so, what can be done to fix it?

Basic troubleshooting from the User's Guide is reset, restart, restore (first from backup then as new). Have any of these steps been tried?
