#3Sorry to say but your best to take it back to an Apple Store, as i said it should still be covered under warranty.

#5your imei and serial number should be on your sim card holder. Ive heard if they confirm the fault in the Apple Store they will just swap out your iPhone straight away. Fingers crossed that'll be the case for you.
