I've bought an iPhone 4 in Brazil in December 2010 and it was unlocked. About two years of use, the buttons started to fail.
I went to NYC in May 2012 and I went to the Apple Store in 5th Ave. Apple "gave me" a new iPhone 4 by the cost of $150.00 USD. But the new iPhone is locked by AT&T and I can use it since I came back to Brazil.
This month, June 2012, I went to Dallas and to Apple Store again. They told me they couldn't do anything, just AT&T. I went to AT&T, and they told me just Apple could do the unlock procedure.
Well, here I am, two months without using my iPhone, because it's locked by AT&T. And I've never had an AT&T contract!
And to make things easy, the guy who did the service in Apple Store at 5th Ave, didn't write the old iPhone data correctly (IMEI, Serial Number, etc.). So... I have the receipt, but the only info it has is the old Serial Number in the top of the recepit.
My question is: how can I unlock my iPhone and use it in Brazil?

Only the carrier an iPhone is carrier locked with can officially unlock the iPhone - or send the unlock request to Apple on the owner's behalf.
In this case that would be ATT regardless what they tell you. It Apple unlocked an ATT carrier locked iPhone without this being requested by ATT, Apple would be violating their contract and agreement with ATT.
You're gonna have to call AppleCare in the states to fix this. You should not have been given an AT&T locked iPhone unless your original phone was in fact originally locked to AT&T. If that were the case, that would mean your original phone was hacked to unlock it. Regardless, you're gonna have to deal with AppleCare.
The information you were given is incorrect. Apple do not unlock iPhones. Only the carrier to which the iPhone is locked can authorise its unlocking. You will need to contact AT&T again, however if you have never established service with AT&T and therefore never been a customer you will not meet their criteria for authorised unlocking. I would contact the Apple Store that exchanged your iPhone, if you original iPhone was officially carrier unlocked they should have replaced like for like. It may be worth trying to get them to exchange your iPhone for an unlocked model.

I really don't know what to do! I had an iPhone 4 bought in Brazil and unlocked. And Apple gave me an locked iPhone when I asked them to fix the iPhone buttons. Now I have kind of "luxury iPod". :-/
Why did he do that? Checking my old iPhone Serial Number and IMEI he could find that it was unlocked!
Well, this week I was in Dallas and I went to an AT&T store. They told me to ask Apple to unlock my iPhone. Apple pushed me to AT&T. And AT&T pushed me to Apple!
I called AT&T support and the attendant told me to send the Apple service receipt by fax. I have the receipt. The guy from Apple wrote the old Serial Number in the service request header. But he didn't write the old IMEI. But in the service request items he wrote the new Serial Number and IMEI.
I'm going to try sending this receipt, though. I hope it works...

The only source of genuine unlocked iPhones in Brazil would
be Apple themselves or Oi who appear not to lock their iPhones
If you purchased from any other source you are likely to have a locked iPhone
probably to AT&T (a gray import ) and was correctly replaced by Apple as locked
Apple will normally only service in the US an iPhone originally shipped in US
not one that was sold new in Brazil .
No international warranty
