I have several alarms configured on my iPhone which are set to go off every weekday morning at 15 min intervals to ensure I get up on time. This morning none of them went off, and when I looked at the phone and touched the screen, all of a sudden they all went off one after the other. The phone was charging, plugged into a Universal Dock Adapter, but this was not the issue as I had had the iPhone plugged into it every night for the past 5 weeks and the alarms all went off fine.
Anyhow, thanks for making me late for work Apple!

I woke up magically at 9:47 for a clinic I had to give at 10!
The alarm is definitely not working. This happened once last
week too, it is bizarre, i triple check it before bed, but it
will just occasionally not go off, but the majority of the time it
does. I cannot figure it out. when i check it in the morning, it says
ON but it never went off....hmmmm any ideas would be appreciated!

Try to do a reset and see if that helps:
Press and hold the home button while at the same time, pressing and holding the button on the top of the iPhone (hold them down for about 10 seconds).
